Partnership for Solutions is bringing together leading organizations representing children, the elderly, various disease-specific organizations and health care professionals with the goal of improving the day-to-day lives of people with chronic conditions.
Led by Johns Hopkins University and The Robert Wood Johnson Foundation, the Partnership provides new information about the challenges faced by those with chronic conditions.
Through original and existing research, the Partnership is working closely with policymakers, business leaders and others to identify promising solutions to improve the care, enhance the quality of life, and provide choices for people with chronic conditions.
